I sincerely wish to become a true Muslim, for many years my thoughts and heart have been Muslim. I, Arthur C. Hammond, son of G. Hammond, deceased, do hereby faithfully and solemnly declare of my own free will that I adopt Islam as my religion, that I worship the One and only Allah (God) alone, that I believe Muhammad to be His messanger, and servant, that I respect equally all prophets, Abraham, Moses, Jesus, etc., that I will live a Muslim life by the help of Allah
